What is a disability certificate?
A disability certificate is an official paper that records a person’s disability. In India it is often needed to apply for pensions, travel concessions, education support, and other schemes. Many people also use it when applying for a Unique Disability ID (UDID) card through the Government of India.
The certificate itself is issued by an authorised medical board — not by an NGO. Lokayatan’s work is to make that path less frightening: awareness, camps, and a helping hand in Lucknow.

How to prepare
1. Write down your questions
Bring the person’s name, age, and what kind of support you are seeking — school, pension, travel, work, or daily living.
2. Keep identity papers together
Aadhaar and any previous medical records help the authorised process later. Do not leave originals in anyone’s care unless you trust the official channel.
3. Come with a companion
A family member or neighbour makes the day easier. Lokayatan volunteers are there as a helping hand, not as a replacement for family.
